Making Aging in Place Easy in Charlottesville

There are many questions to answer as we grow older, and one of the biggest is where to live. For many seniors, staying put creates an invaluable sense of comfort and security. Aging in place is more than just staying in one’s home. It’s remaining connected to the neighborhood and community as well as family, friends, and acquaintances. It’s the familiarity of the local pharmacy, grocery store, and that favorite restaurant. Aging in place is about remaining connected to the people and places who have been part of one’s life for so many years. With planning, and occasionally a bit of assistance, aging in place can be a good, safe option for many seniors. Making a Plan – The Key to Successful Aging in Place When beginning the planning process for aging in place, there can be so many things to consider it can feel overwhelming. That’s where IKOR of Charlottesville comes in – our trained geriatric care managers know where to start. How you want to grow old starts with a basic question, “what do you want?” It may sound like a simple question, but it is one that our clients have rarely explored deeply. With the right guidance from our life care management team, we help our clients develop a road map for how they want to age, and we help them ensure it’s achievable within their means. Making Home Safe Safe aging in place starts by ensuring the home environment is safe. As we age, mobility and agility decline leading to an increased risk of falling and the need for adaptive mobility devices. Initially, aging in place means paying closer attention to items such as area rugs, extension cords, and loose stairs – all of which can lead to accidental trips or falls. Often, brighter lights are needed in the kitchen and bathroom to make it easier to see. Doorknobs and faucets may be changed to levers to make them easier to operate. For seniors with mobility challenges, living in a home may require more extensive home modifications such as wider doorways and a stairlift to ensure comfort or safety. Reaching into the bottom of the washer, or bending down to get clothes out of the dryer, may require different appliances or modifying the laundry room to minimize bending and reaching. At IKOR, we know what investments to prioritize that will make a home safer. Our geriatric care managers are experienced registered nurses with years of clinical experience who often notice little details during a home safety assessment that others miss. Connecting to Services and Professionals  Aging comes with a unique set of challenges. While medical or physical challenges often come to mind first, ensuring one’s financial and legal affairs are in order is just as important. It doesn’t take much searching on the internet to find heartbreaking stories of families who wish they had taken the time to understand their loved ones’ care preferences. In these stories, the families only realize this after their loved one has become incapable of making their decisions. Our experienced patient advocates help guide this conversation raising topics that otherwise might be overlooked. We carefully document these care preferences and help our clients create advance directives. When our clients don’t have up-to-date estate planning documents, we can help them find a lawyer or financial planner specializing in elder law. We remind our clients to review these documents regularly and update them as needed. We can also help guard against undue influence. It isn’t uncommon for a senior to need a bit of extra help around the home. In cases where in-home care would help maintain independence, IKOR of Charlottesville can recommend the best in-home care providers based on our client’s needs. We work with our client and the in-home care company to develop a care plan and ensure everyone is on the same page. Navigating the Medical Maze IKOR’s Registered Nurse Advocates  have a deep understanding of the medical system and senior’s unique needs. We handle it all, from coordinating communication between specialists and primary care physicians to setting up follow-up appointments and arranging transportation as necessary. In addition to Registered Nurse Advocates, our life care management team includes personal needs … Read more

Empowering Geriatric Care Managers with Technology

Living well is about far more than just physical well-being. Overall health – social, mental, financial, and environmental factors – is essential to happiness and life quality. Helping our clients manage all aspects of their health requires organization, collaboration, and occasionally creativity. Our people are at the heart of everything we do, but it’s important to ensure that they have the systems and processes in place to help them succeed. At the center of our operations is our care management software, CareTree. Through this HIPAA-compliant system, we track cases, collaborate as a team, and securely communicate with clients, their family members, and care providers. Delivering Excellent Care Management with the Right Software In 2016, we set out to be the premier provider of geriatric care management and life care management services in the Charlottesville area. From our beginning, we knew there were several key ingredients for success. First, research has shown team-based, interprofessional collaboration is one of the most important factors for improving healthcare outcomes; so, we set out to build a diverse team of experienced professionals. Next, we sought a technology platform to manage all of the information that pertained to our clients’ needs and well-being. We could spend a lot of time explaining the nuances of selecting a heterogeneous information management system, but to be succinct, we needed a system that could securely store many different forms of data – everything from case notes to x-rays; lab results, to legal documents. Finally, our technology platform needed to be simple to use yet powerful enough to help our team find the exact piece of information about a client’s case quickly. The result of this search was a highly customized case management software solution from CareTree. Client-Centered Life Care Management Truly effective client-centered life care management can only happen when all care team members are on the same page. Our software plays a key role in keeping team members up to date and facilitates secure information sharing, not only internally but also with our clients and their designated family members. Client & Family Communication Not all of our clients are tech-savvy, and they don’t have to take full advantage of IKOR’s services. For our clients who are, our software platform gives them access to their information and allows them to send secure messages to their geriatric care manager or other members of their life care team. For adult children living outside the Charlottesville area, our software gives them an “always-on” window into the care their parents are receiving. Our software puts an end to the phone tag game that often occurs between care managers and busy adult children. When we selected software, we wanted to make sure it was easy for everyone to use. Our clients and their adult children often remark that they appreciate the “feed-style” form of information that they’re accustomed to seeing on other popular platforms and the intuitive user experience that makes communicating with the IKOR team a breeze. Our software offers text message alerts to notify clients and family members when an important and relevant update has been posted. Text messages are HIPAA-compliant. If potentially sensitive information is part of the update, the message recipient will be directed to the family portal to log in and review the update. Patient Advocacy Our geriatric care managers are highly experienced registered nurses ready to help our clients navigate even the most difficult medical challenges. To be an effective patient advocate, they need quick and reliable access to our client’s information. Should a crisis strike, our software enables secure access to a client’s vital medical history enabling the geriatric care manager to provide the emergency medical professionals with accurate information at a moment’s notice. Treatment Plans & Medication Analysis Ensuring our client’s best possible healthcare outcomes is one of the key roles for our geriatric care managers. Ensuring treatment plans are in sync and medications are balanced is crucial. Unfortunately, our fragmented healthcare system means specialists and primary care physicians aren’t always on the same page. This can lead to inefficient or contradictory treatment plans and sometimes even adverse interactions between medications. Our software gives our geriatric care managers a bird’s eye view of treatment plans and medications. … Read more

How We Bill for Our Services

When you or your loved one faces changed circumstances related to aging, complicated billing systems are the last thing you need to worry about. We understand, and that’s why IKOR of Charlottesville created the Clear Bill Promise. It’s our way of assuring you that you’ll never be surprised by a service charge and that you’ll understand each item that appears on your monthly bill. IKOR’s Clear Bill Promise IKOR of Charlottesville’s billing is simple and straightforward. Our services are billed hourly in 6-minute increments. Each line-item that appears on your invoice will have an easy-to-understand description. There are no service minimums, and we don’t have hidden fees or monthly retainers. If you don’t use our service during a given month, you won’t receive a bill. Starting Service with IKOR To be the most effective advocate for you and help you age the way you want to age, your life care management team will spend the first few months getting to know you, your preferences, and your future goals. Depending on the services you’ve hired us to perform, we may also gather and organize essential life documents. We’ll collect and organize medical history and care records for clients using our geriatric care management services during the first month. It is not uncommon for your first few bills with IKOR to be higher, as getting to know you takes time. We often say that we’re a surrogate son or daughter, but unlike your actual children, we don’t know everything about you, so it takes us time to get caught up. Monthly Services You will receive an invoice for the previous month’s services at the beginning of each month. We bill for our time in 6-minute increments. The time you are billed is listed on the invoice, along with a clear description of what we did during that time. There are no service minimums, no retainers, and no monthly fees. If you don’t use our service, you don’t receive a bill. That’s the IKOR Clear Bill Promise. Working with Elder Care Professionals We develop a life care management plan for each client based on their needs, preferences, goals, and dreams. Occasionally, we’ll require outside help to fulfill the plan. An outside professional could be a financial planner, an in-home care agency, an elder law attorney, or another elder care professional. Before we engage an outside professional, we’ll present you with several options and make sure you understand how these businesses bill for their services. Once you’ve selected a professional, you’ll pay them for their services directly. At your request, we can liaison with them to make sure everything runs smoothly. Your well-being is our top priority. To ensure that there is never a conflict of interest, or even the perception of one, we never accept referral or finders’ fees from the professionals we recommend. Ensuring a Safe Place to Age Whether you are choosing to age in place or looking for that dream retirement community, IKOR of Charlottesville’s life care management team will help ensure your safety wherever you decide to call home. When we help a client age in place, we typically start with a home assessment. During the assessment, your Registered Nurse Advocate will look for items that can commonly cause trips or falls. They will also help you plan for your future needs. For example, knobs on faucets and doors can be harder to turn as we age, so it might make sense to change those fixtures to levers. Replacing light bulbs with high-output, energy-efficient LEDs can make it easier to see in places such as the kitchen, bathroom, and workspace. Occasionally, more in-depth home modifications or repairs are needed. In these instances, we’ll recommend contractors who have the right skills to make the required modifications, such as constructing a ramp, installing a chair lift, or widening doorways. When we recommend these outside professionals, we’ll give you several options to choose from. We will provide quality assurance and liaison with them to ensure they’re doing quality work that meets your expectations. You’ll pay these professionals directly for their services, and we will get bids to ensure you’re paying a fair price for the work. A Partner to Help You Age … Read more

The Benefits of a Having Geriatric Care Manager

Whoever said, “getting older isn’t easy” certainly wasn’t lying. The marvels of modern medicine have helped us live longer, healthier lives. Still, it has also created a lot of complexity in the healthcare system. As we’ve lived longer and healthier lives, entire industries have formed to cater to a senior’s every desire and help them overcome every challenge – truly, there have never been more choices on how to age than there are right now. Unfortunately, all of this has come at a cost. Seniors and family caregivers routinely report feeling overwhelmed and express a lack of confidence about the choices they’re being asked to make. These feelings often mean seniors put off important decisions that would help them live a better quality of life. Table of Contents Shifting to Proactive Healthcare Management Geriatric Care Managers Guide Seniors Through the Aging Process Fostering Independence While Growing Older IKOR of Charlottesville’s Comprehensive Approach to Geriatric Care Management The Value of Hiring an IKOR of Charlottesville Geriatric Care Manager It’s Time to be Proactive About Healthcare and Aging Healthcare is historically reactive, treating ailments as they come up. But waiting until the point of an emergency is always less than ideal. Individuals who take a reactive approach tend to have worse health outcomes, spend more money on care, and experience increased stress and anxiety – both of which can lead to further health complications.  Being reactive hinders one from being in control of their situation. When a senior suddenly has a reason to need assisted living, for example, a reactive response can leave them feeling negative, hurt, and resentful. They may not adjust to their new living arrangements as well.  Studies have shown that seniors who are less proactive regarding their care are at greater risk for poor health outcomes, including depression. By taking a proactive approach, seniors can regain control of the decision-making process and improve their quality of life.  Proactive Healthcare Management Helps Seniors Live Better By being proactive with potential healthcare decisions, seniors have a say in establishing the quality of life they see for themselves as they age. This is comforting for adult children as they are not left decisions based on assumptions of what their parents want.  For seniors and their families, proactive healthcare management can lead to: Fewer out of pocket medical expenses Better doctor/patient communication Improved independence Safer aging-in-place Increased access to senior-focused resources We’re all familiar with the phrase “a good offense is the best defense,” and the same is true when it comes to one’s health. When selecting a professional to help a senior be proactive with their care, it is important to find a professional with the right training and experience.  Geriatric Care Managers – Expert Guides for Aging Geriatric care managers are advocates for seniors and their families. Traditionally, an advocate thought of as a “supporter, believer, sponsor, promoter, campaigner, backer, or spokesperson.” A geriatric care manager is all of these. Their goal is to educate, support, and speak up for their clients. With a geriatric care manager at their side, seniors are empowered to make informed decisions about their care while planning ahead for future needs. As healthcare becomes more complex, working with a professional to navigate the aging process can be highly beneficial. With a geriatric care manager, families focus on their loved ones while knowing a professional is handling the important details.  Respected Care Managers in Charlottesville At IKOR of Charlottesville, our geriatric care managers are often thought of as surrogate sons or daughters because of the personal roles they take on. Our care managers are highly trained registered nurses who have a deep understanding of both the aging process and seniors’ unique needs.  While the aging process is new to the senior, our geriatric care managers have walked down this path with seniors and their families numerous times. Their knowledge gives seniors the upper hand over aging as they navigate a fragmented healthcare system and deal with the other nuances of growing old. IKOR’s geriatric care managers take time to get to know each client to understand how they want to age. The geriatric care manager then leads the rest of the IKOR life care management team … Read more

Our Coronavirus Response & Plan

At IKOR of Charlottesville, our clients’ well-being is not just a priority, it’s our founding and guiding principle. During this time of heightened awareness and concern regarding the Coronavirus (COVID-19), we wanted to share with you the extra precautions we’re talking among ourselves and with our clients.  New Procedures for IKOR Staff Members: Travel & Large Gatherings – The CDC and public health officials are advising against non-essential travel and recommending everyone avoid large gatherings. We are taking these warnings seriously. Employees must report travel and large events that they have attended to the Managing Director so that their risk of exposure can be assessed, and appropriate action can be taken to minimize the risk of transmitting the virus to others. Personal Care – We are fortunate to have experienced nurses on our team. Our RN Advocates have reviewed preventative health measures with IKOR staff members. These preventative measures include refreshers on proper handwashing, cleaning and disinfecting, and staying home if an individual is feeling sick.  As owner, I have taken special care to make sure my team understands that staying home due to illness will in no way jeopardize their employment. Personal Contact – Our employees will no longer shake hands with or embrace clients, vendors, and others we come in contact with during our day. When practically possible, we will follow the CDC social-distancing recommendation of maintaining six feet of distance between individuals. Conducting Business and Serving Our Clients: Caring for you is our top priority. Our RN Advocates will continue to attend important doctors’ appointments, and most client hospitalizations. We will be pausing all non-essential client visits for both clients living at home and in senior communities. This policy will be evaluated regularly, and non-essential home visits will resume once it has been deemed safe. For clients that are concerned about their prescription medications being refilled, please contact Kathy Allerton, IKOR Case Coordinator. She will work with your RN Advocate to develop a plan. We are in contact with other service providers, including homecare agencies and senior communities, to discuss their plans to address potential caregiver shortages.  Currently, we are not seeing such shortages impact our clients and will be monitoring this subject on an ongoing basis.   Keeping Yourself Safe and Minimizing Your Exposure: Follow proper hand-washing techniques.   Try to maintain a social space of six feet between you and other people. Call ahead before visiting a senior community as many are now limiting visitors. This situation is unprecedented and evolving quickly.  Please seek information only from the most reliable sources such as the Centers for Disease Control and Prevention (CDC). Even as this situation forces us to cut back on in-person client contacts, all of us at IKOR remain deeply committed to the health of each and every one of our clients and families.  Should you have anything you would like to discuss or need extra help or reassurance, please contact IKOR’s Case Coordinator, Kathy Allerton by sending a message or calling 434.202.8887.

Thank you for contacting us.

We have received your inquiry and will follow up with you in the near future. We appreciate your interest and look forward to working with you.

Assisting Executors with Estate Administration

Serving as the estate executor/administrator for a friend or family member is an enormous undertaking and requires considerable time, knowledge, and running around. When a non-attorney first agrees to serve as a loved one’s executor, we find that most don’t fully comprehend the work involved and can quickly become daunted by the responsibility they now face. IKOR can share the work by contacting utility companies to terminate services, delivering or retrieving documents from the County/City Clerk’s office, inventorying and securing estate assets, arranging and overseeing realtors and appraisers, drafting communications to heirs, and a host of other non-legal tasks.

Concierge Services

We know life is busy and that the day-to-day tasks that absorb our time can seem never-ending.  Let IKOR take some of the burden of life’s general demands off your shoulders.  Whether it’s arranging a hair appointment, dropping documents off at your attorney’s or tax preparer’s office, or shopping for the perfect outfit for your granddaughter’s wedding, our concierge services combine reliability, personalization, and ease.
